Health Information Management (HIM)
 

The Health Information Services Department, also known as Medical Records, is committed to protecting patient privacy and to providing you with records in a timely manner. 
 

Office Hours:

The department is open 8AM – 4:30 PM, Monday through Friday.
 

How do I request a copy of my medical records?

Print a Consent Form for Release of Information. Complete the form, making sure to include a daytime phone number and the patient’s signature. You may either mail this form or submit it in person.

How long will it take to process my request?

It depends on how much information is requested. It may take 48 to 72 hours or more to complete your request after we receive the consent form. We will do our best to accommodate special requests that require faster processing. You will be asked to show your picture ID.
 

Do I have to pay for copies of my records?

Records needed for continued or follow-up medical care will be sent directly to the physician’s office, free of charge. Charges will apply for records needed for personal use.
 

How can I request a certified copy of birth certificates?

All birth certificates are maintained at the Dodge County Courthouse in the Probate Judge’s office. You may contact them at 478-374-3775.
